The brief

A free, no-signup screening tool for Pakistan: the PHQ-9 depression questionnaire and GAD-7 anxiety screen, in Urdu and English.

It had to give an instant score against the published cut-offs while being honest that a screening score is not a diagnosis.

What we built

The questionnaire itself, an instant score with its severity band, an explanation of where each total sits, a section on the limits of a number, and a large Q&A.

No sign-up is needed to take the test.

Design & technical notes

A calm, clinical but kind design with clear type, generous spacing and nothing alarming.

Fully bilingual with proper Urdu typography, built to load quickly on any phone.
